چکیده
Mediastinal pancreatic pseudocyst is rarely a complicat ion of pancreatit is. Both the syndrome itself and diagnostic chest radiograph can easily be confused with hiatus hernia. It is a challenge for radiologists to distinguish a mediastinal pancreatic pseudocyst from a mediastinal cystic lesion on CT (computed tomography) axial imaging. Here, we present the case of a 51-year old man with severe epigastralgia, vomiting and nausea. Chest radiograph and axial CT imaging revealed a posterior mediast inal cyst ic mass which could be mistaken as hiatus hernia. Using MDCT (multidetector computed tomography) coronal and sagittal images, it is easy to distinguish between a mediastinal lesion (e.g., hiatus hernia) and the pancreas, and thus it is possible to distinguish a mediastinal pancreatic pseudocyst. Successful resolution of a mediastinal pseudocyst and pancreatic pleural effusion by endoscopic nasopancreatic drainage.
CONTEXT A mediastinal pseudocyst is an unusual complication of acute and chronic pancreatitis. The ideal form of management is controversial, and various successful therapeutic interventions including surgical resection, internal or external drainage, and non-operative radiological drainage techniques have been described. Successful resolution of a mediastinal pseudocyst with endoscopic transpa...
متن کاملFocal Tumor-like Splenic Extramedullary Hematopoiesis: a case report
Extramedullary hematopoiesis (EMH) has been reported in various organs. However, its manifestation as a focal tumor-like splenic lesion is rare. Here, we report a case of EMH in an 85-year-old female presenting as a well-defined intrasplenic mass of 12 cm on sonography and computed tomography. Splenectomy was performed under the impression of splenic tumor, and intrasplenic focal EMH was diagno...
متن کاملMediastinal extension of a complicated pancreatic pseudocyst; a case report and literature review
BACKGROUND Mediastinal pancreatic pseudocyst is a rare complication of acute or chronic pancreatitis. CASE PRESENTATION This case report describes the management of a difficult case of pancreatic pseudocyst with a mediastinal extension in a patient having chronic pancreatitis. Different management strategies were used until complete resolution of this complex pseudocyst occurred using open su...
متن کاملComputed tomographic appearance of internal herniation through the sigmoid mesocolon.
The case of a patient with surgically proven internal herniation of a loop of ileum through the sigmoid mesocolon is described. This 66-year-old man presented clinically with acute lower abdominal pain and an elevated white blood cell count. A computed tomography (CT) scan showed a thickened bowel loop with "bird-beak" appearance in the pelvis, centered towards the medial side and lying aside t...
متن کاملSuccessful treatment of mediastinal pancreatic pseudocyst by pancreatic head resection.
CONTEXT A mediastinal pseudocyst is an unusual and rare complication of acute and chronic pancreatitis. CASE REPORT The authors report the case of a 41-year-old male patient with a documented history of chronic pancreatitis who developed a pancreaticomediastinal fistula with mediastinal pseudocyst, which was successfully treated by pancreatic head resection (Frey). CONCLUSION Though the cho...
